Many TRS forms can be filled out on your computer with Adobe Acrobat Reader. After printing a form, write your name and TRS member ID at the top of every page and make sure nothing was cut off at the edges. If you submit a paper form, you must sign it by hand and either mail or fax it to TRS, or securely upload it through the TRS website. The only electronic signatures accepted by TRS are those completed through MyTRS. Please call 800-223-8778 if you would like us to mail you a form instead.
